Step 1:
Please download the registration form, fill it out, and e-mail to padco@rochester.rr.com.

Step 2:
Please either send alternate payment to Park Avenue Dance Company, 15 Vick Park B, Rochester, NY 14607 or pay online for Class(es) and Annual Registration using PayPal*.
Please enter the class you wish to take under instructions.

Step 3:
Please download the Liability Release form, sign it, and either e-mail in a pdf format to padco@rochester.rr.com or bring with you to class.
